Try Ed Pillinger, he offers exactly the service you mention, at least for clarinet and saxophone. http://www.pillingermouthpieces.co.uk/
I don't know about Tarogato mouthpieces but I am sure he would rise to the challenge. I haven't used his pieces myself but there are some forum members who had something re-created - may have been a Link or similar - and are very happy with it.

Hey Steve,
If you contact Ed Pillenger would you keep me in the loop ? My Tarogato mouthpiece has a chip in the tip which I 'fixed' using wood filler then carefully sanded down. It made the mouthpiece nominally playable but it doesn't seem like a very good long term fix to me.
Mine doesn't appear to ever have been refaced so facing measurements might be able to be taken from mine if yours has been compromised to the point where it might be difficult to re-create them.
Mine has a 'gozzinia' shank as you may recall from the old forum. As I recall, yours has a standard clarinet style tenon.
Maybe he would consider some kind of a 'twofer' deal ???
